New Patient Information
Dr. Barger is accepting new patients at this time. We can usually get you in the same day. To save time, you can click on the "New Patients Forms" selection to the right of this paragraph to download and print the new patient forms. This will enable you to fill the forms out ahead of time and bring them with you to your first visit. If you would like us to bill your Health Insurance for you, please bring your insurance card with you. Dr. Barger is "In Network" with most health insurance companies including Anthem Blue Cross/ Blue Shield, Humana, Aetna, Med Ben, United Health Care(UHC), United Medical Resources(UMR), Medical Mutual, to name a few. Dr. Barger also accepts Medicare, Indiana Medicaid, Ohio Work Comp(BWC), and patients that have been in Auto Accidents. Our office accepts cash, check, VISA, Mastercard, and Discover.
